Release of Liability for Complimentary Massage Services with Pure Life Therapy

  1. Acknowledgment of Services
    I understand that the massage therapy session I am receiving is being offered as a complimentary service, free of charge. This service is intended for relaxation and general well-being and is not a substitute for medical treatment or professional healthcare.

  2. Voluntary Participation
    I acknowledge that my participation in this complimentary massage session is entirely voluntary. I am under no obligation to accept this service.

  3. Medical Disclosure
    I certify that I have disclosed all relevant medical conditions, allergies, injuries, or sensitivities to the massage therapist prior to receiving the service. I understand that failure to disclose such information may increase the risk of adverse effects during or after the session.

  4. Assumption of Risk
    I understand that massage therapy involves physical contact and manipulation of soft tissues, which may result in temporary discomfort or soreness. I voluntarily assume all risks associated with this session, including but not limited to, the risk of injury or exacerbation of pre-existing conditions.

  5. Release of Liability
    I hereby release and discharge Pure Life Therapy, its employees, agents, and representatives from any and all liability, claims, demands, or causes of action arising from or related to the massage session, including but not limited to, injury, illness, or adverse effects.

  6. Consent to Touch
    I understand that massage therapy requires hands-on techniques. I provide my consent to touch and the application of professional massage techniques as agreed upon before the session.

  7. No Guarantees
    I acknowledge that no guarantees or warranties have been made regarding the outcomes or effects of the massage therapy session.

  8. Legal Agreement
    I have read this release of liability in its entirety and fully understand its terms. By submitting the form below, I agree to the conditions set forth and release the provider of this service from liability.
